Qualified Social Worker - Hospital Discharge

Experienced Qualified Social Worker required to work in the Hospital Discharge Urgent response Team.
Roll is fast paced with a focus of prevention of going into hospital. Cases held for 5 days before being referred over.

3 month rolling contract
£32ph
ASAP Start
Duty in office only – Monday to Friday 1 week a month
Rest of the time can be remote/ flexible on working location + visits
Based just outside of Tower hill station – South Norwood.

Must have previous hospital discharge experience to be considered.
