to complete Care Act Assessments, Reviews, Safe guarding, care coordination under CPA Framework. 
Manage a caseload of 25 clients and be part of a Duty Rota, which may include screening, triaging and new referrals.  
To be part of an integrated Mental Health and wellness Team. 
£33.42ph
