Adult Safeguarding Social Worker, Hammersmith and Fulham  
Pay - £35 per hour.  
To engage with individuals, carers and families to assess and intervene, and using professional judgement, employ a range of interventions promoting choice, control and independence. 
The role holder will possess developed specialist knowledge of social work practice in adults with a learning disability or mental health needs and be registered with Social Work England. They will assist individuals, carers and families to develop support plans to meet the outcomes identified, using a strength-based, asset-based approach and family, friends and community options where appropriate and eligible needs are identified. 
The role holder will deliver services that are co-produced with residents by actively encouraging involvement and ownership of the support plan but also take a leadership role where appropriate and will give clear explanation of the use of Personal Budgets and Direct Payments so that informed decisions can be made.
