Adult Social Worker, Hospital Team, Hillingdon  
Senior Practitioner Role 
Specialise in multi-disciplinary assessments with health -DST’s Duty manager role – to manage daily discharges via D2A pathway and Pathway 3 placements, to triage cases alongside the Hillingdon HUB and out of borough hospital referrals. Attending conference calls with health professionals and being able to deliver results. Supervising staff, allocating cases, management of stepdown beds. Safeguarding Manager Role, attending weekly meetings with safeguarding lead on progression of investigations. Carry a small caseload of complex cases We need an enthusiastic person to join the hospital team, to take part in managing safe hospital discharges in a pressured environment and delivering results. Ensuring performance targets are being met with in the team. 
£38 per hour
