Adult’s Social Worker, Hospital Discharge, Somerset 
Pay - £38 per hour  
On site Acute Hospital Social Worker to support hospital discharges in line with intermediate care process so will need knowledge of mental capacity act and discharge planning.
Will need to be willing to work onsite daily
