Social Care Assessor – Hospital Discharge Team (HDT) Enfield 
We are seeking an experienced Social Care Assessor to join Enfield’s Hospital Discharge Team (HDT), supporting the swift, safe and successful discharge of Enfield residents from hospital settings. You will work closely with NHS colleagues as part of a fast-moving, supportive Team, coordinating adult social care input to enable timely discharges across discharge pathways. The role involves ward-based work, proportionate Care Act practice, liaison with families and hospital staff, coordination of care packages and practical problem-solving to unblock delays. You will ensure clear, outcome-focused documentation and effective handovers to longer-term teams. This is a well-supported team with strong management oversight and peer support. Hybrid working is available, with regular presence required at North Middlesex Hospital, Barnet General Hospital and St Michael’s Hospital. 
£23.87 per hour
