Later Life Service
Pay rate £35 per hour
Vitalis Social Care are recruiting for an experienced and qualified Adults Social Worker to join a later life team in St. Helens.
Job Responsibilities: -
The Later Life service in St Helens works with individuals aged 67+ with age related frailty and mental health issues such as dementia. This is a busy team with the social workers completing Care Act assessments on individuals, devising care plans and reviewing services.
The social workers have a busy caseload and are expected to manage a high number of referrals. It is expected the social workers will have a good understanding of current legislation and government policy related to care of the elderly and an understanding of the Mental Capacity Act and DoLS legislation.
This post will be to predominantly complete hospital discharges for out of borough hospitals which will entail visiting out of borough hospitals to complete Care Act assessments, Mental Capacity assessments, best interest assessments and attending case conferences as and when required.
Good support structures are in place and regular supervision is provided by team managers or advanced practitioners.
