Children’s Family Coordinator, East Riding of Yorkshire
Pay rate, £18.68 per hour
Vitalis Social Care are recruiting for an experienced Family Coordinator to work directly with and deliver targeted and coordinated interventions to families with children aged 0-19 years (or 25 with SEND) who have complex and interacting needs.
Job Role and Responsibilities: -
Supporting families against a co-produced, strength-based assessment and family support plan. You will work with partners to best identify ways to support children and families, including effective information sharing. The goal is to improve outcomes for children and families and help them to be resilient, make a positive change and reduce the risk of a need for greater intervention.
