Vitalis Social Care are recruiting for an experienced and qualified Adults Social Worker to join an Integrated Discharge
Job Role and Responsibilities: -
Experienced social worker required to work in the Integrated Discharge Team (Contact Cares) based in Whiston Hospital. This will entail assessing patients and other tasks associated with facilitating hospital discharge, to support patients with the support they require to return home or to be discharged to another setting. The role is very fast paced and high pressured and experience in this area would be preferred. This will be a full-time post (37 hours per week) between the hours of 8.45am and 5.15pm. There will likely be a mixture of working in the office and working from home on a rota basis and will involve ward visits to assess patients where necessary.
