Occupational Therapist Assistant – Supportive Community Role

Job Details:
  • Employment Type: Temporary (18/09/2025 – 27/11/2025)
  • Hours: 37 hours per week (Monday–Friday, 09:00–15:00)
  • Pay Rate: PAYE: £15.83 per hour | Umbrella: £20.17 per hour
  • Location: Newport
Key Responsibilities:
  • Conduct assessments to identify the needs of individuals and carers for moving and handling or specialised transfer equipment to ensure safe care delivery at home.
  • Develop comprehensive moving and handling plans and risk assessments to guide Care and Support teams, community health professionals, and domiciliary care providers (internal and external).
  • Perform person-centred functional assessments and recommend equipment and low/medium-level home adaptations to enhance independence.
  • Assess individuals’ circumstances regarding access to suitable parking to support the annual Disabled Parking Bay Scheme allocation panel.
  • Collaborate with multi-agency teams to ensure cohesive delivery of care and support plans.
  • Maintain accurate records and ensure compliance with health, safety, and data protection policies.

Requirements:
  • Essential: Experience in conducting assessments for moving and handling or equipment provision in health or social care settings.
  • Essential: Strong communication skills and ability to work collaboratively with individuals, carers, and multi-agency teams.
  • Essential: Knowledge of person-centred care, health and safety, and risk assessment processes.
  • Desirable: Familiarity with Welsh social care policies or experience with home adaptations.
  • Must have: Eligibility to work in the UK, Criminal Convictions Declaration, CV; Confidentiality Agreement required upon offer acceptance.
