📍 Location: West Yorkshire (Hybrid – 2 days in office)
💷 Pay Rate: £35 per hour

🧩 What You’ll Be Doing:

• Assess and support individuals with disabilities, children with complex needs, and older people in their homes
• Recommend and coordinate provision of specialist equipment and adaptations
• Prevent hospital admissions and facilitate safe discharges
• Work closely within multi-disciplinary teams and with partner agencies
• Write clear, concise reports to support intervention planning and service delivery

✅ About You: To succeed in this role, you’ll need:

• A recognised Degree or Diploma in Occupational Therapy, with HCPC registration
• A minimum of 2 years’ post-registration experience
• Experience in at least one of the following areas:
  
  • Major adaptations
  • Moving & handling
  • Paediatric equipment/complex needs
• Strong report writing and assessment skills
• Proven ability to work collaboratively in multi-agency settings
• Sound knowledge of legislation relating to disability and safeguarding
• A valid UK driving licence and access to a vehicle (Essential Car User allowance applies)
